Payments can be made via Cash, Credit Card, or your HMO’s Letter of Authorization (LOA). To speak to an Admitting Officer, call 523 8131 local 2050 or 2051. Please take note that charges for procedures vary depending on the chosen room type.
You may choose from the following Room Types:

If you have an LOA from your HMO/Insurance or your company, your admission may be done in your pre-reserved room. To reserve a room, call our Admitting Officer.
One companion may stay with you overnight. Kindly minimize the number of your visitors especially children for their own safety. Our visiting hours are from 9:00AM to 9:00PM.
In case you want an extra room for your family, we can refer you to the Pearl Manila Hotel for a special rate. The hotel is linked to the hospital via bridge way on the second floor.
Our Hospital, in order to ensure medication safety, does not allow you to bring in your own medicine. Medicines for your stay will be provided through our Pharmacy.
After the admitting papers are signed, a patient ID bracelet will be applied on your wrist. For your safety, do not remove this. Our nurse will unfasten the bracelet during discharge.
FORMAL ECONOMY (SSS and GSIS member)
- Certification from the employer (Part IV of CF-1)

- Member Data Record (MDR)
- Summary or Certification of PHILHEALTH contribution (Six (6) months prior to confinement if without operation)
INFORMAL ECONOMY MEMBER (Individually Paying Program/Self-Employed)
- Member Data Record (MDR)
- Philhealth ID or any valid ID
- Clear copy of Official Receipts covering Six (6) months prior to confinement if without operation
OFW MEMBER
- Member Data Record (MDR) with validity period
- Clear Copy of Official Receipt/s covering qualifying contribution
INDIGENT/ SPONSORED MEMBER
- Family Health ID card with validity period
- Member Data Record (MDR)
LIFETIME/PENSIONER MEMBER (GSIS and SSS member)
- Philhealth ID for non-paying member
- Member Data Record (MDR)
- Member Data Record (MDR) wherein the name of the dependent is stipulated
- Spouse : Marriage Contract with Civil Registry number
- Children below 21 years old: Birth Certificate of dependent with Civil Registry Number
- Parent 60 years old and above: Copy of Birth Certificate of member with Civil Registry Number & Photocopy of Senior Citizen ID

While admitted, you may consult with a Financial Counselor for updates and concerns about your billing. You may reach a Financial Officer at (02) 523 8131 loc 2067 from 8:00am through 6:00pm.
DISCHARGE PROCEDURE
I. Your Attending Physician must write the order for discharge in the medical chart.
II. The Nurse will inform the Billing Section of your discharge order who in turn will prepare your final hospital bill.
III. The Nurse will inform you when the Philhealth Office and Cashier are ready for you. Please bring the following documents:
- Professional Fee Form (from the Nurse)
- PhilHealth Form 2 (from the Nurse)
- Completed PhilHealth CF1 Form (given during Admission)
IV. Settle your account at the Cashier. You will be given:
- A White Slip to surrender to the Nursing Station
- A Blue Slip to surrender to the Security Guard
V. Our nurse aide will asist you to the front door.
If you require a taxi, please inform our Customer Service Officer.
1. By car from EDSA
- Go straight all the way until you hit Roxas Boulevard
- Turn right at Roxas Boulevard then go straight all the way until U.N Ave.
- Turn right at U.N. Ave. go straight until you cross Taft Ave. then turn right into the hospital.
2. By car from SLEX
- Go straight northbound until Osmeña Highway, then turn left at Quirino Ave.
- On Quirino Ave. just go straight until you hit Taft Ave.
- Turn right at Taft ave.
- Then turn right at U.N. Ave.
3. By MRT-LRT
- When at the MRT go down at the last station southbound (EDSA – Taft).
- Take the LRT EDSA station then go down at U.N. Ave.
4. By LRT2-LRT1
- When at the LRT2 go down at the last station southbound (Recto Station)
- Then take the LRT line 1 (Doroteo Jose) and go down at U.N Ave.
The entrance to our parking is located on General Luna Street beside Jollibee. A bridge-way connecting to the hospital is located on the second floor.
